Quilt Registration Form
Please submit individual forms for each quilt.

Deadline for Submission:  September 10, 2011

All quilts need the owner's name permanently displayed somewhere on the quilt.  All quilts, including miniatures, must have a 4" sleeve installed on the top of the back of the quilt.  Quilts will not be accepted for display without a sleeve.

In the event that there are too many quilts to display, please indicate your priority for hanging with the following notation:  if you enter a total of 5 quilts, your first priority would be marked below as 1/5.  Do not include charitable quilts that you have already given to the chairs of those committees.
